1) You can only sign up one attendee at a time.

2) If the ceremony dates you are looking at are coming up in the next two weeks and you would like to check on spaces available for a group of two or more people, please email us at ceremony@aya.guide.

3) If you have any questions or concerns prior to signing up, please review our Ceremony Questions page or open discussion with us by emailing us at ceremony@aya.guide.

4) To sign up for this event, please select the dates you’d like to attend and complete the short registration form below. If so, you can join our Waiting List by emailing your phone number and the specific dates you are interested in attending to us at ceremony@aya.guide.

5) You will need to supply your ceremony donation to complete the sign-up process. You will receive an email after supplying your donation confirming your sign-up. Please be sure to review and complete the steps in your confirmation email to finalize your ceremony registration.

Have you ever had gastric bypass surgery? *
Are you pregnant, or do you think you could be? *
Diagnosed or undiagnosed, have you ever had any issues with the following mental-health conditions: Bipolar Disorder, Borderline Personality Disorder, Dissociative Identity Disorder, Schizophrenia or Schizoaffective Disorder? *
I understand that I will be asked to fill out a Health Form and Questionnaire and that I must provide this information in a timely fashion in order to maintain my ceremony reservation. *
I understand that I may be contacted if more information is needed. If for any reason we decline your registration, your donation will be promptly refunded. *
I understand that taking pictures, filming, or any type of recording, without permission, is not allowed during the course of the ceremony weekend. *
I understand that my ceremony donation is non-refundable, and that registrations declined by us will be promptly refunded. *
I promise to review the A.Y.A. Ceremony Preparation Guidelines and to prepare my body, mind, and spirit to participate in the sacred space of ceremony. *
